Family Practice Physician (MD, DO) for a position in Santa Rosa, CA.
The Family Practice Provider will
collaborate with their assigned care team to deliver high-quality primary care
services to patients of all ages, following accepted standards of medical
practice. Responsibilities include responding to patients' medical/health
issues, which involves history-taking, diagnosis, investigations, treatment,
and referrals as needed, all while maintaining confidentiality and
impartiality. The role also involves promoting health education alongside other
health professionals and sharing night and weekend telephone calls with fellow
staff clinicians. Staying current with medical developments, new drugs,
treatments, and medications is an integral part of the role. This position
revolves around delivering family-oriented medical care services within a
Patient-Centered Medical Home Model.
Clinic

postpartum patients as well as urgent care patients (suturing, splinting,
etc).

Monday - Friday,
8am-5pm. Flexible scheduling options including 5 8-hour days, 4 10-hour days, 3 weeks on/1 week off

Graduate of an

The emergency room operates 24-hours a day and Sexual Assault Nurse Examiners (SANE) responders are always available. Diabetes prevention treatment is provided both individually and in group settings, i.e. kitchen creation/eating healthy programs are available. A nutritionist works as part of this team to monitor, track, and create healthy living and lifestyle recommendations. Podiatry services are available on a scheduled basis through contract specialists. Physical therapy and well-child visits are conducted in an outpatient setting, by appointment. Public health nursing works with the Hopi Tribe Community Health Workers to monitor, follow-up, and provide individual and group health interventions. Tribal Behavioral Health Services are also available 24 hours a day. It serves a rural and underserved community in Polacca, AZ. Ideal candidates are those who will consider picking up at least 6-12 shifts per month on an on-going basis.

Fixed-Wing Aircraft might take 1-2 hours, including the time for transfer to and from airports.

Typical productivity (based on usual mix of new
and established patients) will be 12 to 16 patients per day. In general, 2 to 4

Time for interpretation of diagnostic tests
(e.g., EEG) is provided. However, administrative time is generally not provided

Locations of care/schedule (specific days of

Parker PHS Indian Hospital provides comprehensive general medical care and pediatric services to the local community.
